Mixed care team provides the best results – Västerbotten Courier

Older people affected by stroke or hip fracture cared best if all involved health professionals and expertise collaborate in specific groups, so-called interdisciplinary teams. It follows from two reports from the Swedish Council on Technology Assessment.

The reports show that coordination between staff and experts give patients more opportunities to return to a normal, active life compared with conventional care. Most likely, the method is also more cost effective.

The group around the patient coordinate discharge from the hospital the same group is also responsible for the rehabilitation of the home.

Every year, 25,000 people stroke in Sweden, of which 80 percent are over 65 years old. The incidence of hip fractures is approximately 17,000 per year. About two-thirds are women and the average age is 82 years.
